HF-REACH (Heart Failure – Remote Engagement, Access and Care at Home) is a digitally enabled, nurse-led clinical trial designed to improve access to structured heart failure care for people living in regional, rural, and remote communities.
Patients are supported remotely by a specialised cardiac nursing team who monitor symptoms, weight, blood pressure, heart rate, and medication adherence. Nurses provide education and self-management support, identify potential deterioration early, and escalate clinical concerns to the patient’s GP or cardiologist when required.
For participating general practices, HF-REACH is designed to support rather than add to existing chronic disease care. The central research team coordinates study screening, consent, remote monitoring and research activities, while the patient’s local clinician remains responsible for their usual clinical care.

We invite GP practices in regional Queensland to partner with HF-REACH to connect interested regional practices and clinicians with the program and help expand access to heart failure research and digitally enabled care closer to home.
