- The Chronic Conditions Primary Care Package.
- Submitting an Expression of Interest (EOI) to develop a place-based model of chronic conditions management for your practice (step two). This facilitated codesign process will occur early in 2024. Small grants are available to support protected time for practices successful in the EOI process to participate in codesign workshops.
- Participating in a Request for Proposal (RFP) tender process for funding to pilot that model of care for two years (step three). Pilots will commence mid-2024.
Background
Chronic conditions are the biggest health challenge facing Australia today. Lifestyle changes and a reduction in infectious diseases mean that chronic conditions are responsible for most of the current burden of ill health.
Primary care settings play a critical role in preventing and managing chronic conditions. Management is an ongoing process that benefits from a coordinated, patient-centred, and team-based approach. Current funding models and service design and systems can make integrated, person-centred care difficult to achieve, but reform is occurring.
NQPHN is committed to supporting primary care providers to enhance their chronic condition management. The Chronic Conditions Primary Care Package provides general practices with a practical opportunity to consider how they can strengthen their model of chronic condition care and be well placed to engage with the Strengthening Medicare reform agenda.
Eligibility
Eligible: Private general practices in the Cairns and Hinterland, Townsville, and Mackay Hospital and Health Service regions that are accredited (or working toward accreditation) under the National General Practice Accreditation Scheme.
Ineligible: Primary Care Clinics run by Hospital and Health Services or the RFDS, and Aboriginal and Torres Strait Islander Community Controlled Health Services.
Submitting an ROI
If your practice meets the eligibility requirements and you would like to participate or learn more, it is important that you submit an ROI by 2pm Friday 3 November 2023.
This simple process only requires you to provide the name and location of your practice, number of GPs, accreditation status, and nominated contact person and contact details.

After submitting an ROI
Mark time in your diary to attend the Market Briefing. A copy of the presentation along with questions (de-identified) and answers from both sessions will be provided to all practices who submit an ROI. An invitation to submit an EOI will be sent to your nominated contact.
Please note: In the event of a poor response in any region, NQPHN reserves the right to invite any practice to submit an ROI and/or extend the closing time in that region.
