  • Identify patients at risk of hospitalisation or poor outcomes
  • Monitor chronic disease indicators

  • Support MyMedicare registration and Practice Incentives Program (PIP) Quality Improvement (QI) reporting.

Northern Queensland Primary Health Network (NQPHN) recently developed and implemented a Chronic Conditions Management Continuous Quality Improvement program using Primary Sense that:

  • identifies patients with chronic conditions and their complexity scores
  • enables proactive care planning
  • flags medication safety alerts to reduce the risk of adverse events
  • racks overdue Care Plans and Reviews to support timely follow-up and billing
  • monitors MBS item utilisation to improve practice efficiency
  • supports MyMedicare registration to enhances continuity of care.

Impact on chronic disease management

During the Proactive Chronic Conditions Management (ProCCM) program, practices using Primary Sense saw measurable improvements resulting in:

  • increased nurse-led care planning
  • improved patient engagement through recall and reminder systems
  • better use of MBS items for chronic disease management
  • enhanced confidence among nurses and GPs in managing complex patients.

Practices reported a peak in nurse MBS item utilisation during the months when coaching support was provided – demonstrating the power of combining data insights with workforce development.

Using Primary Sense to recall patients for care plans
