Free Chronic Disease program

The Foundations of Chronic Disease program is aimed at a range of health professionals working within Aboriginal and/or Torres Strait Islander Health or have Aboriginal and/or Torres Strait Islander clients.

The program provides students with the skills and knowledge required to provide foundation chronic disease education, understand the burden of chronic disease and the impact on Australia, identify chronic disease models, and the tools and resources used in managing chronic conditions in the primary health care arena. It also allows the student to build and embed foundational knowledge to allow advancement to further learning workshops. 
 
It is a fully online program consisting of blended learning including knowledge, information, articles, learning videos, and self-reflection activities. The program includes detailed information on the determinants of health, Indigenous health, chronic disease frameworks, the patient journey, health literacy, and chronic conditions. 

Upon successful completion of the program students should have the ability to: 

  • understand the burden of chronic disease
  • identify priority groups
  • understand their role in Close the Gap 
  • understand the patient journey and link to coordinated care, cultural safety, health literacy and communications styles 
  • identify the need for prevention and the use of prevention tools 
  • understand care coordination.
