Aboriginal and Torres Strait Islander Health Check Templates Feedback

The Department of Health have endorsed the recently released Aboriginal and Torres Strait Islander health check templates as the recommended templates to support high quality annual health checks.

These templates were developed by the National Aboriginal Community Controlled Health Organisation (NACCHO) and the Royal Australian College of General Practitioners (RACGP) in collaboration with a large working group and published in 2020. Adaptation to local needs is encouraged.

The key changes reflected in the templates are:

  • inclusion of and increased emphasis on patient priorities and goals
  • consistent age-appropriate evidence-based elements
  • greater inclusion of the determinants of health
  • prompts to support follow-up of identified health needs.

The NACCHO-RACGP Partnership project team are keen to hear your feedback on the updated health check templates and are requesting your participation in a brief survey open from 9 March until 1 April.

Your feedback will support the team to understand what it takes to get these health check templates into practice and what other innovations can support quality health checks and primary healthcare for Aboriginal and Torres Strait Islander people.

From 12 April until 11 June they are also keen to work with ten primary healthcare teams to test getting the updated templates into practice to understand what enables, complicates, or prevents using these in practice. These templates sit on the project’s resource hub along as part of a suite of resources including Good Practice Tables which provide suggested activities for all members of the practice team, with each activity linked to accreditation. And don’t forget, they are always keen to hear from you at that address with any further feedback or ideas you may like to share to support high quality primary healthcare that is valued by Aboriginal and Torres Strait Islander patients.
