Secondary use of Primary Sense data form

This form allows general practices to opt out of having their de-identified Primary Sense data used for secondary purposes. Practices can notify NQPHN if they do not want their data used for a specific secondary purpose or for any secondary use at all.

Please note that opting out of data usage for specific or all secondary purposes will not affect your Primary Sense data sharing arrangements with NQPHN.
