Designing Care: Lessons from community led integrated health projects

Join us for an online conversation exploring what it takes to design and deliver primary health and social care that is truly place-based, equity-driven and responsive to community voice.

Primary health care is facing convergence of pressures and possibilities, rising chronic disease, funding pressures and persistent inequities in access and outcomes. At the same time, there is a growing recognition that the future of primary care must be community-led, culturally grounded and build around relationships rather than transactions.

Drawing on inspiration from organizations such as The Alex Community Health Centre in Canada, The Purple House in Australia, and The Australian Centre for Social Innovation (TACSI), this session will examine how locally governed, culturally anchored models of care can address complex health and social challenges while strengthening dignity, belonging and health outcomes. Together, we will explore what these examples mean for health care commissioning, system reform and the future of primary care leadership.
