The PHASES (Preventing Heart Attacks and Stroke Events through Surveillance) Webinar Series supports general practices across northern Queensland to strengthen the prevention and management of cardiovascular disease.
These practical, online sessions help primary care teams better identify and manage patients at risk of heart attack and stroke, using tools such as Primary Sense™, the Australian CVD Risk Calculator, and quality improvement approaches aligned with routine general practice workflows. The webinars support a whole‑of‑practice approach, engaging GPs, nurses and practice staff in achievable, staged improvement activities.
The series aligns with NQPHN’s delivery of the PHASES project, supporting practices through education, data‑driven quality improvement and integration with existing accreditation and PIP QI requirements. Practices are encouraged to attend as a team and apply learnings with support from NQPHN’s PHASES team.
