Introduction to PHASES

Introduction to PHASES (Preventing Heart Attacks and Stroke Events through Surveillance)

PHASES is a statewide screening, recall and treatment program supporting general practice to prevent and manage cardiovascular disease (CVD). It combines clinical tools such as Primary Sense™ and the Australian CVD Risk Calculator, practice-level quality improvement support, and a consumer campaign to drive earlier intervention, optimise care, and reduce the burden of disease.

Speakers

  • Professor Kim Greaves: Clinical Lead, PHASES: Cardiologist & Epidemiologist, Sunshine Coast Health
  • Zoltan Bourne: General Practitioner – Maleny Doctors

Learning objectives

  • Understand the scale and impact of cardiovascular disease in Australia, including its disproportionate impact on priority populations.
  • Gain clarity on PHASES aims, scope and expected outcomes, including how it addresses CVD within the community.
  • Learn how Primary Sense™ supports early identification and optimisation of management through clinical auditing.
  • Identify practical steps to collaborate with your PHN on project implementation and evaluation.

Bring the whole team – everyone plays a role in improving primary health care and outcomes for patients at risk.
