The Outreach Generalist Health Worker, Rheumatic Heart Disease (RHD) works with the RHD team in caring for RHD clients and their families and assists in delivering education and support to families at risk of RHD.

Wuchopperen is a Community Controlled Aboriginal Health Organisation providing holistic health care services to the Aboriginal and Torres Strait Islander people of Cairns.

  • Casual Positions
  • Based in Cairns and Edmonton
  • Pay rate $56.88 - $68.43 per hour (inclusive 25% casual loading)

The Registered Nurse is responsible for working with clinical staff to improve the standard of health of Aboriginal and Torres Strait Islander people through the provision of culturally valid, holistic primary health care services.

The successful applicant will hold a nursing degree together with current registration with the Australian Health Practitioners Regulation Agency (AHPRA) and a Queensland immunisation endorsement. Recent practical experience within a community health setting is highly desirable.

Mental Health and Alcohol and Other Drugs

The Tablelands and Mareeba regions is one of six trial sites for DBS across Australia and is expected to be operational in early 2025. To inform implementation, Northern Queensland Primary Health Network (NQPHN) has been engaging with community members and service providers in the Tablelands and Mareeba regions.

Distress Brief Support (DBS) is a community-based approach to support people experiencing stress or distress, made up of two elements:

  • Community engagement points that provide a compassionate response to people, along with the offer of connection to further support 
  • A short-term support team that provides up to 3 weeks of practical support, and connections to broader services and supports.

What have we learnt?

DBS Findings 2024

  • Community members tend to reach out to family, friends, and social networks or groups when they are experiencing stress or distress, rather than formal services.
  • There is a vast range of community groups already established that represent potential community engagement points.
  • Whilst the DBS model does not include payment for community engagement points, it does include training, coordination support, and debriefing, and we need to find an agency who can do this work, perhaps one with experience in volunteer coordination.
  • The workers in the short-term support team need to be locally based, rather than commuting from Cairns.
  • We need to pay attention to rural towns in the region, not just the central locations of Mareeba and Atherton.
  • Smaller community organisations are interested in the trial, but don’t have much experience in completing formal application processes for these sorts of activities.

Tender information sessions are scheduled to provide an opportunity for NQPHN to present information about the tender process and for potential applicants to ask questions.  Please register for a session below.

Distress can take a wide variety of forms. Distress can be an unpleasant emotion, feeling, thought, condition, or behaviour. 

Emotional distress may include unpleasant feelings, symptoms, or experiences which may result from a particular event, or life circumstances. 

Situational distress may be challenging or a mix experience of thoughts, emotions, bodily sensations, or behaviours, which may result from life events or situations such as grief, a change in health status, relationship breakdown, financial, or workplace or employment difficulties. 

Over time, NQPHN will be seeking expressions of interest from individuals and community groups who would like to be an identified community engagement point. Community engagement points will not be paid but will be provided training and ongoing support and may include social clubs, service organisations, public facing workers, family playgroups, Men’s Sheds, mother’s groups, and so on.

In October 2024, NQPHN is planning to launch an application process to identify agency/s (incorporated community organisation, not-for profit organisation, registered charity etc) to be funded to coordinate community engagement points and provide support through training and debriefing.

In October 2024, NQPHN is also planning to launch an application process to identify agency/s to employ the short-term support team working with people who are experiencing distress.

Timely conversations are crucial when caring for a person with dementia at their end of life

Caring for someone with dementia as they approach the end of their life can often feel lonely and isolating. 

However, engaging in conversations about death and dying can help families and caregivers feel more prepared and supported.  

According to Sarah Fox, the Project Coordinator for the HammondCare Last Days Dementia workshops, these discussions are crucial. 

To make it easier to have these conversations, HammondCare, in partnership with Northern Queensland Primary Health Network (NQPHN), recently ran a series of workshops, which were conducted by palliative care nurse specialists both face to face in Cairns, Townsville, and Mackay, and online. 

Ms Fox, who is deeply passionate about initiating discussions about dementia and dying, said it was important to provide families and caregivers with the necessary tools and support during this challenging time. 

“Having timely conversations is paramount when caring for a person with dementia,” Ms Fox said.  

“As the disease progresses, it can be difficult to identify symptoms and ascertain advance planning preferences.   

“Dying is the same process for all, but for a person dying of or with dementia, unique symptoms can be experienced.” 

Attendee feedback from the Last Days Dementia workshops in northern Queensland has been positive.  

Ms Fox said 94 per cent of surveyed participants felt more “confident to start conversations to make the person’s final days meaningful, comfortable, and fulfilled. 

“We know that people caring for someone coming to the end of their life can sometimes feel lonely,” she said. 

“By attending the Last Days Dementia workshops, we hope to make a difference by helping them feel prepared, while understanding their options, and allowing time for conversations and reflection.  

“The workshops are delivered by palliative care professionals giving people the practical knowledge and tools they need to manage this process and also relieves the uncertainty and confusion.” 

Each workshop consists of five areas that cover demystifying death and dying, future planning, what to expect, rituals, rules, and regulations, and grief and self-care.  

Comments from participants include: 

  • “It was very valuable, well-organised, and excellent content, the facilitator had relevant and relatable experience and knowledge. I left with loads to reflect on and think about.”  

  • “I came not knowing too much and left with so much information. Excellent presenters.”  

  • “The topic of death and dying is usually a feared one. It was very refreshing to have a ‘normal’ conversation about it. Our society needs many more of them. I also learned a lot of practical information.”  

  • “The presentation was very enjoyable. I feel this will help me improve the conversations I have with my clients, their families, and/or carers more openly but also give me more confidence to start conversations with my own family.” 

Ms Fox said carers who took part in a Last Days Dementia workshop felt better equipped to navigate the little-understood and difficult journey.  

“Our hope is that we can raise awareness of this important topic and enable this conversation to gain momentum,” she said. 

“We believe the community must own its central role in end-of-life and bereavement care, with health professionals advising, supporting, and contributing to empower people to make informed decisions. 

“The workshops aim to strengthen death literacy for individuals, health care professionals, and the community.”
