Apunipima is a community-controlled health service offering great opportunities to grow and develop.

  • Location: Cairns, servicing Cape York communities.
  • Status: Full time Fixed Term (until 30 June 2024).
  • Salary: $105,607-115,208 per annum plus super.

About us  Apunipima Cape York Health Council is a community controlled Aboriginal and/or Torres Strait Islander health service, which provides culturally appropriate, family centred, primary health care services to the communities and people of Cape York.

About the opportunity The Integrated Team Care (ITC) Lead manages the Apunipima ITC program. The position leads the ITC team to ensure individuals with chronic conditions and complex care needs receive systematic care planning, access to services, support and follow-up in accordance with patient care plans and program guidelines.

It is a condition of employment for this position that the employee be vaccinated against COVID-19 in accordance with the organisational policy requirements.

Pre-employment screening for vaccination against Vaccine Preventable Diseases will be conducted for the successful applicant.

About you To be successful in this role you will have:

  • a nursing degree or diploma in Aboriginal and Torres Strait Islander Primary Health Care
  • current registration with AHPRA as either a Registered Nurse or Aboriginal and Torres Strait Islander Health Practitioner
  • demonstrated experience in the management of a range of chronic conditions and the use of chronic condition care plans and pathways
  • experience as team lead/manager with understanding of the historical and contemporary issues that impact on Aboriginal and/or Torres Strait Islander people and their communities, particularly in Cape York
  • ability to communicate sensitively and effectively with Aboriginal and/or Torres Strait Islander people in accordance with community protocols and customs
  • current Queensland drivers licence.
