Closing the Gap with Communication of Medication Reconciliation at Admission and Discharge Events

Cairns and Hinterland Hospital and Health Service (CHHHS) implemented Closing the Gap with Communication of Medication Reconciliation at Admission and Discharge Events (CoMRADE-CTG) after securing funding through the Connected Community Pathways initiative. Closing the Gap with Communication of Medication Reconciliation at Admission and Discharge Events (CoMRADE-CTG) is a pharmacist-led medication management service to support patients self-identifying as First Nations with high medication needs to safely transition back into the community.  

CoMRADE-CTG liaises with primary care providers in the community to ensure patients are receiving support around medication management . Our purpose is to reduce hospital admissions and reduce medication misadventures, this is completed through the support of medications access, medication liaison, and Hospital Outreach Medication Reviews (HOMRs) which is an alternate pathway to Home Medication Reviews (HMRs) and Residential Medication Management Review (RMMRs). HOMRs are comprehensive medication reviews that aim to identify medication-related problems and provide recommendations or strategies as required.
