A Registration of Interest (ROI) process is underway to allow eligible general practices to indicate their potential interest in participating in the Chronic Conditions Primary Care Package.

The Chronic Conditions Primary Care Package will support private general practices in the Cairns and Hinterland, Townsville, and Mackay Hospital and Health Service regions to develop place-based models of chronic conditions management that target their local populations.

Submitting an ROI is an essential first step in a three-phased approach to participating in the Chronic Conditions Primary Care Package. Practices submitting an ROI will be invited to attend the Market Briefing, scheduled for 8am and 1pm on Wednesday 8 November (same presentation at both sessions). The Market Briefing will include information on:

  • The Chronic Conditions Primary Care Package.
  • Submitting an Expression of Interest (EOI) to develop a place-based model of chronic conditions management for your practice (step two). This facilitated codesign process will occur early in 2024. Small grants are available to support protected time for practices successful in the EOI process to participate in codesign workshops.
  • Participating in a Request for Proposal (RFP) tender process for funding to pilot that model of care for two years (step three). Pilots will commence mid-2024.

General practices can submit an ROI from 2pm Tuesday 24 October until 2pm Friday 3 November 2023. Submit an ROI here

We encourage you to register your interest if you would like to learn more about the Chronic Conditions Primary Care Package.  Submitting an ROI does not commit you to participating further in the procurement process BUT it is essential that you have submitted an ROI if you wish to participate in later stages.

Chronic conditions are the biggest health challenge facing Australia today. Lifestyle changes and a reduction in infectious diseases mean that chronic conditions are responsible for most of the current burden of ill health. Primary care settings play a critical role in preventing and managing chronic conditions. Management is an ongoing process that benefits from a coordinated, patient-centred, and team-based approach. Current funding models and service design and systems can make integrated, person-centred care difficult to achieve, but reform is occurring.   NQPHN is committed to supporting primary care providers to enhance their chronic condition management. The Chronic Conditions Primary Care Package provides general practices with a practical opportunity to consider how they can strengthen their model of chronic condition care and be well placed to engage with the Strengthening Medicare reform agenda.

Eligible: Private general practices in the Cairns and Hinterland, Townsville, and Mackay Hospital and Health Service regions that are accredited (or working toward accreditation) under the National General Practice Accreditation Scheme.

​Ineligible: Primary Care Clinics run by Hospital and Health Services or the RFDS,​ and Aboriginal and Torres Strait Islander Community Controlled Health Services.​

If your practice meets the eligibility requirements and you would like to participate or learn more, it is important that you submit an ROI by 2pm Friday 3 November 2023. This simple process only requires you to provide the name and location of your practice, number of GPs, accreditation status, and nominated contact person and contact details. ​

Mark time in your diary to attend the Market Briefing. A copy of the presentation along with questions (de-identified) and answers from both sessions will be provided to all practices who submit an ROI. An invitation to submit an EOI will be sent to your nominated contact.

Please note: In the event of a poor response in any region, NQPHN reserves the right to invite any practice to submit an ROI and/or extend the closing time in that region.

Digital capability grants program
The Inca Integrated Care Platform is a comprehensive shared health record and care planning solution that connects patients and their care team. 

With up-to-date access to patient information, Inca allows providers to give the best and most informed care to patients, drive clinical efficiency, and maximise patient engagement.

Practices can:

  • See all patient data in a single place.
  • Keep patients and care teams on the same page.
  • Identify care gaps faster.
  • Reveal actionable insights.
  • Integration - Inca syncs with all major practice software to automatically populate key demographics and clinical information.
  • Care plans - Inca creates Medicare-compliant GPMPs and TCAs. Guided templates have been developed in consultation with the RACGP and include input from key advisory agencies such as Diabetes Australia.
  • Shared health record - Real-time access allows care team members to communicate easily and share reports, case notes, and other patient details. This visibility allows for better-informed decision making which can lead to improved patient outcomes.
  • Patient engagement - Patient engagement is a key contributor to the success of care plans. Inca provides patients with access to their personalised care plans and allows them to communicate with their care team via the Inca web portal. The Meditracker App also allows patients to access their health record at any time.

INCA integrates seamlessly with general practice software consolidating information including diagnosis, medication, immunisation, and pathology, as well as allied health and specialist reports and notes, and hospital data. There is also the ability to include information from Bluetooth devices that a patient uses at home within a patient record. This presents a true multidisciplinary view of the patient health history to those providing care.

Any documents added to the Inca record from other team care providers can be downloaded into your practice software patient file.

NQPHN are funding all general practices in the region. Other team care professionals do not need the platform installed to access a patients care plan.

New service will support adults with mental health challenges
Cairns Head to Health will be a free service providing extended hours of service, with no appointment or referral needed
Cairns Head to Health will be a free service providing extended hours of service, with no appointment or referral needed.

Codesign for the new Cairns Head to Health service is underway with more than 50 community members and service providers turning out for the initial community consultation session to have their say.

Northern Queensland Primary Health Network (NQPHN) and Cairns and Hinterland Hospital and Health Service (CHHHS) held the session in early September at the Cairns Colonial Club Resort, in Manunda, to ensure this new service is designed to meet local needs.

The session was well represented by local service providers, hospital and primary healthcare clinicians, people with lived experience, and community members keen to help codesign the service.

Attendees were provided with the opportunity to work through four activities, based on the Double Diamond approach to human-centred design.

Cairns Head to Health will provide a safe and welcoming space for people to receive compassionate support for a variety of needs that can be supported outside of the hospital setting and emergency department.

It will be a free service providing extended hours of service, with no appointment or referral needed.

The service is a joint initiative of NQPHN and CHHHS and further codesign activities will be undertaken throughout the remainder of 2023.

A procurement process will be undertaken in early 2024 to identify a service provider/s to deliver the Cairns Head to Health service.
