The Northern Queensland Primary Health Network (NQPHN) privacy policy ensures that the collection, use and disclosure of personal information complies with the specific requirements of the Privacy Act 1988 and associated Australian Privacy Principles (the legislation).

NQPHN commits to implement the practices, procedures, and systems that will ensure NQPHN complies with the legislation.

The legislation regulates the way the NQPHN collects, stores, uses, and discloses personal information.

NQPHN will implement practices based on the principles of openness and transparency when assisting individuals understand how their personal information is managed. 

NQPHN recognises the rights of individuals to have their personal information managed in such a way as to make it secure and protected, and accessible to them upon request.

NQPHN commits to a complaint handling process to manage privacy complaints in an open, transparent, and timely manner. 

Information on how NQPHN complies with the legislation is outlined below.

 

Directly related business functions or program activities.

We are an independent, not-for-profit organisation funded by the Australian Government to commission primary health care services to help northern Queenslanders live happier, healthier, longer lives.

We collect information from various sources to assess and understand the health needs of the region, engage with stakeholders and collect personal information for our business activities such as recruitment, finance and contract management.

We collect de-identified information authorised for secondary purposes; this information contains no personal information. 

Information provided for secondary purposes can only be provided to us with your consent. If you have any concerns regarding the provision of de-identified information, please contact your healthcare/ service provider.

We do not collect personal clinical (health) information. If we receive personal information of any kind from a third party, the receipt of that information will be handled in accordance with our data breach procedure.

We will not collect personal information from our website, MS teams or other meeting platforms, social media, email lists, registration, or feedback/ enquiry forms for direct marketing purposes unless the information provided by you is provided for that specific purpose.

  • You have the option of dealing with us anonymously or under pseudonym and you will be advised of any potential consequences of doing so.
  • You will not be excluded from participating in the activities of the organisation if you request anonymity or use a pseudonym.

However, we are not required to provide these options if it is not practical or is authorised by law to deal with identifiable individuals.

To protect your personal information from misuse, interference, loss, unauthorised access, modification or disclosure, we have established a Data Governance and Privacy framework incorporating user access and network security.

A Privacy Champion, Privacy Officer, and Data Governance Committee safeguard a culture of privacy, governance and continuous systems improvement.

Our workers are educated in privacy obligations at the commencement of employment and will be informed of changes to legislation and/ or policies and procedures as they occur.

The personal information we collect is used and disclosed only for the purposes for which it is collected. We will not disclose your personal information unless we gain your consent to do so or if it is required by law.

Information is only disclosed upon request and is authorised or declined through our data request procedure.

We will only transfer personal information to someone who is in another State or foreign country if:

  • It is required by law.

Or you consent to the transfer and the recipient is bound by legislation that is substantially similar to the Privacy Act 1988.

Individuals have the right to request access or change to be made to personal information NQPHN holds about them.

Unless a complaint can be dealt with immediately to the satisfaction of both parties, we will provide a response to you in line with our complaints management procedure.
